BSA has a new Health and Medical Record form. Here is the link to print the form:
The new form has parts A, B and C.
Every scout, leader and occasional weekend parent must fill out parts A and C!
This is a must for anyone participating in any activity up 72 hours.
Part B - is for anyone participating in any activity over 72 hours (summer camp)
This must be filled out by a physician or nurse practitioner and is valid for only 1 year, regardless of age or activity.
(No more class II's that were good for 3 years)
Please everyone to fill out the new form, part A and C. If you need to go to the doctor to be in compliance for summer camp, please have them fill out the new form, part B.
If you recently went to the doctor and had them fill out the old form (class II or class III) I will be able to accept that for summer camp this year.
If you are going to Sea Base, have the doctor fill out the Sea base form and the new part B too.
As of Dec 31, 2009 the old Class I, II, III form will no longer be valid.
